Families and Friends:
We want to inform you of the following confirmed cases of COVID-19;2 Employees and 1 Resident.
The safety of our residents and staff is our top priority. If your loved one are diagnosed with COVID-19, we will notify you directly via telephone. To reduce the spread and impact of COVID-19, we continue to follow guidance from the Centers for Disease Control and Prevention (CDC) and the Center for Medicare & Medicaid Services (CMS).
We are staying in close communication with local and state health officials to ensure we are taking the appropriate steps under the current circumstances, including:
· Enhanced infection control precautions, such as becoming vaccinated, increasing the availability of hand sanitizer, and more frequent cleaning and disinfecting of high-touch surfaces.
· Screening residents and staff for COVID-19 symptoms, as well as anyone else who must enter our facility.
· Testing staff and residents for the virus causing COVID-19 for those that are showing signs and symptoms.
· Providing and requiring facemasks for staff and visitors.
· Providing facemasks for residents, if tolerated and following CDC guidance.

July 1st, 2022
The facility currently has 13 residents and 2 staff members as well as 1 agency staff who tested positive for COVID-19
and the facility is following the CDC and CMS guidelines.
The facility is currently completing 3 times a week of testing for all staff and residents until 14 days pass without a new positive case. The facility is also completing contract tracing for residents and staff on such units where the positive residents reside and positive staff members work.
We are open for indoor visitation and our recommended visiting hours are 9:00 am-8:00 pm. Masks are required to be worn at all times and all other CDC guidelines (social distancing) must be adhered to.
March 31st, 2022

The facility is currently completing weekly testing for staff who are not up to date on their vaccination of COVID-19 as is required per the CDC and state guidelines.
December 15th, 2021
We were informed this week of an employee that tested positive for COVID outside in the community. That employee has been removed from the schedule and will return once the quarantine requirements set forth by the CDC have been met.
I want to assure you that when we learned of this we immediately began facility-wide testing of all residents and staff. We are pleased to announce that everyone tested negative. Testing will be ongoing every 3-5 days for all residents and staff for the next 14 days to ensure the safety and wellbeing of everyone.

November 12th, 2021
Per CMS guidelines updated 11-12-21 the facility is now fully open to unrestricted indoor and outdoor visitation.
Although there is no limit on the number of visitors that a resident can have at one time, visits should be conducted in a manner that adheres to the core principles of COVID-19 infection prevention and does not increase risk to other residents. Visitors should not walk around the facility, rather go directly to the resident's room.
Core Principles of COVID-19 Infection Prevention
• Visitors who have a positive viral test for COVID-19, symptoms of COVID-19, or currently meet the criteria for quarantine, should not enter the facility. All visitors will be screened prior to visiting residents.
• Hand hygiene (use of alcohol-based hand rub is preferred)
• Face covering or mask (covering mouth and nose) and physical distancing at least six feet between people, in accordance with CDC guidance.
We encourage visitors to be tested on their own before coming to the facility (e.g., within 2–3 days).
CMS strongly encourages all visitors to become vaccinated and facilities should educate and also encourage visitors to become vaccinated.
